Cardiac Rehabilitation Program
Braintree Rehabilitation Hospital's Cardiac Rehabilitation Program provides patients with the comprehensive medical care, intensive therapies, and support services they need to reach their maximum level of independence.
Goals
Designed for individuals with cardiac related diagnoses, the program maximizes functional abilities and promotes optimal physiologic adjustment by providing:
- Individual assessments of cardiovascular response to exercise in conjunction with a progressive exercise/activities program.
- Patient/family education about the disease process and lifestyle modifications
Program Components
Braintree's Cardiac Rehabilitation Program adheres to standards established by the American Association of Cardiovascular and Pulmonary Rehabilitation. It uses an outcomes-based approach to rehabilitation.
Components include:
- Medical Management by an internist and physiatrist in consultation with a cardiologist
- Nursing Assessment/Ongoing Evaluation
- Multidisciplinary Education Program
- Comprehensive Individualized Rehabilitation, including therapeutic exercise and functional retraining
- Dietary Consult
- Case Management/Discharge Planning
- Additional Services Upon Referral (i.e., vocational rehab, speech therapy)
- Comprehensive Outpatient Services
