Athletic School Physicals

Several times a year physicals are required in order for a high school athlete to participate in school sponsored sporting events. The medical staff at Braintree Rehabilitation Hospital are available to provide those physicals. We can also provide a "LAST MINUTE" service for those athletes who have been unable to obtain their physical in a timely manner. The charge for this service is $50.00.

Please note in all cases involving a student under the age of 18, a consent form, signed by a parent or legal guardian is required. If a parent/legal guardian is unable to accompany a student to a "Last Minute" physical, the consent can be hand carried by the student or faxed to 781-380-4809.
