Cognitive-Neurological Rehabilitation: Community Re-Entry

We offer children and families a well managed, coordinated delivery of services based on the child's developmental and cognitive level. Our community based model allows for flexible, creative "real world" experiences to address a child's many needs. Knowledge about their personal goals for school, life style and daily activities are incorporated into the treatment we provide. As a "resource connector" we link you to the variety of services available out in the community.

The child must:

  • agree to actively participate in the program
  • have the ability to attend the service in a community setting independently (transportation may be arranged)
  • not be abusive to themselves or others
  • be independent in mobility, feeding and ministering to own needs (or have access to a Personal Care attendant-PCA)

Children will participate in one or more of the following evaluations:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Neuropsychological
  • Therapeutic Recreation (Preferred setting in the survivors home environment with support network present)
